An unannounced inspection was initiated on 2-5-2026 and completed on 3-4-2026 in response to a complaint received by the licensing office on 2-2-2026 relating to allegations of Staffing and Supervision and Programs. The inspector reviewed 1 staff records on-site and electronically on 2-6-2026. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
