Inspector Notes

An unannounced monitoring inspection was conducted on February 8, 2024, from 3:30 p.m.through 4:40 p.m. Upon arrival, there were 19 children, ages 5 through 10, in the care of 3 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 children and 3 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review the center failed to have one working battery operated flashlight and one batter operated radio.

Evidence:

1. The radio and flashlight were observed.
2. The batteries in the flashlight and radio were not working.
3. The director confirmed the radio and flashlight were not operable due to batteries not working.
