Inspector Notes

An unannounced monitoring inspection was conducted on February 7, 2024, from 4:00 p.m. through 4:45 p.m. Upon arrival, there were 22 children, ages 5 through 10, in the care of 3 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 children and 3 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation the center failed to ensure one child was in actual sight and sound supervision of staff.

Evidence:

1. One child, age 5, was observed leaving the cafeteria for dismissal.
2. The child was observed walking down the hallway to be picked up by the parent who was waiting outside the exit door.
3. Staff were inside the cafeteria and were not in actual sight and sound of the child.
4. The director confirmed staff were not in actual sight and sound of the child during dismissal.
