Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 9, 2024, as a part of the licensure period. The on-site inspection began at 3:00 p.m. and ended at 4:10 p.m. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 staff. The inspector reviewed 5 children?s records and 4 staff records. This inspection included document review tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each staff member shall submit a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

The screening for Staff #4 was completed 5 months after hire.
