Inspector Notes

An announced initial inspection was conducted on 08/17/2023. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, emergencies, and nutrition. A total of 1 applicant record was reviewed. The center was evaluated to determine the license capacity and the emergency supplies and policy/procedures were reviewed. A recommendation for licensure will be made upon the correction of the non-compliance with the applicable standards. The inspector arrived for the inspection at 9:25 am and departed the center at 11:45 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on staff interviews, the center failed to develop written playground safety procedures that shall include provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children and method of maintaining resilient surfacing.

Evidence: Applicant 1 stated that the playground safety procedures had not been developed.
Plan of Correction: The playground will not be used until the necessary changes are made. The playground safety plan will be developed at that time.

Based on staff interviews, the center failed to have a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and relocation, shelter-in-place, and lockdown.

Evidence: Applicant 1 stated that they did not have a written emergency preparedness plan.
Plan of Correction: The plans are being established today.
