Inspector Notes

An unannounced monitoring inspection was initiated on 05/23/2025 and completed on 06/02/2025 in response to a self-report received by licensing office on 05/23/2025 relating to administration and staffing and supervision. The inspector reviewed 1 child?s record and 2 staff records on-site. The inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 05/22/2025, Child 1 (age 18 months) was left alone on the centers outside playground for approximately 4 minutes. Child 1 was not within the actual sight and sound supervision of staff during the time that they were alone on the playground.
Plan of Correction: The staff will be retrained. The center will implement a more through name to face protocol.

The center shall maintain compliance with the center?s own policies and procedures that are required by the standards.

The center?s required playground safety plan states that teachers are required to take a head count of students before departing the playground to head back inside from outdoor play. On 05/22/2025, Staff 1 and Staff 2 left Child 1 (age 18 months) on the outdoor playground alone for approximately 4 minutes. The staff did not conduct a head count of the students prior to departing the playground to head back inside.
