Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/25/2025. The on-site inspection began at 10:25 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 52 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 08/25/2025.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-500-B
A nonabsorbent surface for diapering or changing shall be used.

The diapering surface in Class 1 had tears and cut creating an absorbent surface.
Plan of Correction: Will discard the old one and purchase a new one.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

The authorization for Medication 1 has been expired for 20 days and was still at the center.
Plan of Correction: Will speak to the parent today and request a new form.
