Inspector Notes
An unannounced 60-day inspection was conducted by the licensing inspector on Tuesday, November 28, 2023, with the center director. The inspection began at approximately 10:15 AM and concluded at 12:00 PM. There were 25 children present, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence:
1. All classrooms did not have a current written list of all children?s allergies, sensitivities, and dietary restrictions.
2. Staff stated that there were children with allergies and food preferences, but there was no documentation in the classrooms.
3. A allergy, sensitivity and dietary restrictions list was posted in the lunch room area uncovered.
Evidence:
1. Two prepared bottles in the refrigerator were labeled with children?s name but were not dated.
2. Four prepared bottles of formula were on the counter labeled with children?s names but were not dated.
Evidence:
1. The record for child #2 (DOE 11/13/2023) and child #4 (DOE 11/6/2023), did not contain documentation names of persons authorized to pick up the child.
2. The record for child #5 (DOE 7/23/2022), did not contain the name, address, and contact number for 1 of 2 emergency contacts.
