Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on September 25, 2023. There were 8 children ages 17 months to 4 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 3 children?s records and 3 staff
records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

EVIDENCE:
Center did not disclose "in writing" the personnel qualifications to the parents/guardians.
