During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site code compliance inspection was initiated and completed on 03/31/2026. The on-site inspection began at 9:30 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 21 children present with 6 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in Code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Violations
13There were 9 children with 1 staff in Class 4 while on the playground. Child 1 (age 22 months) was identified as the youngest child in the group. The required ratio for children from 16 months to 22 months is 1 staff per 5 children; 2 staff were required for this group of children.
There were 4 children in Class 1; the attendance for Class 1 had not been taken to document the arrival of each child as it occurred.
There were 6 children with Class 3 but only 5 children were marked present.
There were 8 children with Class 4 but only 7 children were marked present.
Child 6 (age 16 months) has a diagnosed food allergy to a food that is available at the center. There was not a written allergy care plan available for Child 6.
The most recent immunization record for Child 1 (age 22 months) was dated 10 months ago.
The most recent immunization record for Child 2 (age 20 months) was dated 7 months ago.
The most recent immunization record for Child 3 (age 2 years), was dated 19 months ago.
There were multiple containers of disinfecting wipes sitting on shelves in various rooms in the center that were accessible to children. There was an unlocked disinfectant spray in the diapering area of Class 1.
Upon arrival to the center, Child 5 (age 3 years) was observed at the top of an exterior staircase, leading to the second story of the center. The center staff were unaware that, while the children were playing on the playground, Child 5 had left the group and climbed up the stairs to the second story of the center. The child was not within the actual sight and sound supervision of staff during this time.
Staff 1 was in the restroom of Class 4 changing a diaper and assisting two other children while they were using the restroom. The other three children in the group were not within the sight and sound supervision of the staff during this time.
Staff 1 was in the restroom area of Class 4 while the children were sleeping; the children were not within the sight and sound supervision of the staff.
The children in Class 1 were alone in the classroom when Staff 4 left the class to go to the kitchen.
Three children in Classroom 1 did not wash their hands with soap and running water after diaper changes.
Staff shall wash their hands with soap and running water before and after changing a diaper.
Staff 4 did not wash their hands in between changing three children?s diapers.
The portable sink in the diapering area of Class 1 did not have running water.
The diapering surface shall be cleaned with soap and water and sanitized after each use.
The diaper changing surface in Classroom 1 was not cleaned and sanitized after each use; three children?s diapers were changed without the diapering surface being cleaned and sanitized.
Medication 5 was administered on one occasion (11/13/2025) without written permission from the parent.
Medication 6 was administered to Child 6 (age 16 months) on two occasions (03/02/2026 and 03/04/2026) without written permission of the parent. The manufacturer?s instructions for Medication 6 required physician?s instructions for administration to children under the age of 2 years; physician?s instructions for the administration of Medication 6 was not available.
Child 7 (age 11 months) was in a highchair without the protective belt securely fastened.
