Inspector Notes
An unannounced complaint inspection was conducted on 02/08/2024 in response to allegations received by the licensing office on 01/30/2024 in the areas of staff qualifications and transportation. There were 55 children, ages 16 months -4 years, and 13 staff providing direct care and supervision to children present during the inspection. Staff were interviewed and a total of 3 staff records were reviewed. Two inspectors arrived for the inspection at approximately 8:25 am and departed approximately 10:10 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider

The information gathered pursuant to an investigation of the complaint supports the allegation and the complaint was determined to be valid.

5REPEAT VIOLATION
Based on record review, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence
1. Staff 4 confirmed the hire date for Staff 1 was 02/01/2024. No eligibility letter was available for review. Staff 1 was observed working at the center on 2/8/2024.
2. Staff 2 had a documented hire date of 02/05/2024. No eligibility letter was available for review. Staff 2 was observed working at the center on 2/8/2024.
REPEAT VIOLATION
Based on record review, the center failed to ensure that an employee must not be employed until the agency has the person?s completed sworn statement or affirmation.
Evidence:
Staff 4 confirmed the hire date for Staff 1 was 02/01/2024. No sworn statement was available for review. Staff 1 was observed working at the center on 2/8/2024.
Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: Staff 3 had a documented hire date of 02/06/2024. No tuberculosis screening was available for review. Staff 3 was observed providing care to children.
REPEAT VIOLATION
Based on record review and staff interviews, the center failed to ensure that staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: There was no documentation of orientation available for review for Staff 1. Staff 4 confirmed the hire date for Staff 1 was 02/01/2024. Staff 1 was observed working at the center on 2/8/2024.
REPEAT VIOLATION
Based on observations and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.
