Inspector Notes

An unannounced inspection was initiated on 08/07/2024 and completed on 08/07/2024 in response to a complaint received by the licensing office on 08/01/2024 relating to allegations of Administration and Special Care Provisions and Emergencies. The inspector reviewed 1 child?s record and 2 staff records onsite. This inspection included document review, and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

COMPLAINT RELATED
Based on review of medication, medication authorization, medication logs and discussion with staff, the center failed to ensure that the center administers drugs only to the child identified on the prescription label in accordance with the prescriber?s instructions pertaining to dosage, frequency, and manner of administration.

Evidence:
1. Prescriber?s instructions state that Medication 1 should be administered to Child 1 at 2:00 pm daily.
2. On 6/7/2024 and 7/24/2024 Medication 1 was not administered to Child 1 in accordance with prescriber?s instructions. Child 1 was in attendance on 6/7/2024 and 7/24/2024.
Plan of Correction: Provider will administer medication in accordance with prescriber?s instructions.

REPEAT VIOLATION
Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.

Evidence: The record for Child 1 did not contain the following information: name address and phone number of two designated people to call in an emergency if a parent cannot be reached (no address for both designated people).
Plan of Correction: Provider will obtain this information.
