Inspector Notes

An unannounced complaint inspection was initiated on 11/26/2024 and completed on 12/03/2024 in response to a complaint received by the licensing office on 11/25/2024 relating to allegations of staffing and supervision. The inspector reviewed 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff 2, who has been employed over 90 days, has not yet completed this course.

COMPLAINT RELATED
When children are in ongoing mixed age groups, the staff to child ratio and group size applicable to the youngest child in the group shall apply to the entire group.

On 11/25/2024, for over 50 mins, Staff 1 and Staff 2 were supervising 18 children, ages 16 months to 10 years. The required staff ratio for this group of children was 1:5 and maximum group size was 15. Four staff would be required.
Plan of Correction: Immediately increase staff presence in classrooms and play areas to meet
state-required ratios, Hire additional staff. Supervisors will conduct daily checks on staff-to-child ratios during all operating hours to ensure continuous compliance. Each classroom supervisor will document the child-to-staff ratios at the start and end of each day. Turning kids away ensures that each child is in a safe, adequately supervised
environment.

The inspector observed a child from the toddler group drinking from a cup while walking around the room and a child from 2-year-old group drinking from a cup while walking around the room.
