Inspector Notes

An unannounced inspection was initiated on 03/12/2025 and completed on 03/13/2025 in response to a complaint received by the licensing office on 03/09/2025 relating to allegations of staffing and supervision and special care provision and emergencies. The inspector reviewed 1 children?s record and 1 staff record on-site and electronically on 03/13/2025. This inspection included document review, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to special care provisions and emergencies and are documented on the violation notice issued to the program.

Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff #1 was alone with a group of ten, three-year-old children and was not certified in CPR or first aid.
Plan of Correction: Provider will ensure each group of children has a person certified in CPR and First Aid.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children?s serious and minor injuries and the record shall include the required components.

The injury record for Child 1 did not contain time parent was notified and how parent was notified.
Plan of Correction: Provider will revise the current injury form or use the model form.
