During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 04/29/2025 and completed on 05/09/2025 in response to a complaint received by the licensing office on 04/25/2025 relating to allegations of staffing and supervision, physical plant and special care provisions and emergencies. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/20/2025. A POC submitted after this date will not appear on the public website
Violations
8The sworn statement for Staff 3, who has been employed over 10 days, was dated five days after their first day of employment. The sworn statement for Staff 4, who has been employed over 10 days, was not dated.
The results of the central registry search for Staff 4, employed over 2 months, had not been obtained at the time of the inspection and Staff 4 was working alone with a group of 7 children.
Disinfectant spray was observed on the counter by the sink in the older 2?s room
Uncovered electrical outlets were observed in the older 2?s room and the 3?5-year-old room.
Children?s hands shall be washed with soap with running water or disposable wipes before and after eating meals or snacks.
In the 4-year-old room, children were observed going from the carpet to table to eat lunch without washing hands or using a disposable wipe.
SYSTEMIC VIOLATION
At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.
Staff 6 was alone with a group of four children, ages 21 months- 2 years, and was not certified in CPR or first aid.
The center shall maintain a written record of children?s serious and minor injuries and the record shall include the required components.
The injury record for Child 1 did not contain: staff present, date and time parent notified, future action to prevent recurrence of injury, staff and parent signatures or two staff signatures and how parent was notified.
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff 3, who has been employed over 10 days, and Staff 5, who has been employed over 3 months, did not have any completed reference verifications on file.
