Inspector Notes

An unannounced monitoring renewal inspection was conducted on March 20, 2024 from 12:20pm to 2:25pm the inspection was concluded on March 20, 2024. There were aproximaltey 20 children present and 4 staff members. Children were taking a nap and transitioning into naptime. Outdoor area was inspected. Five staff records and five children records were reviewed. Licensing Inspector
reviewed indoor areas, supervision, activities, equipment, emergency evacuation and sheltering-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat
Based on record review, the licensee did not ensure each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence :
1- Staff # 2 hire date was 01/22/24, the TB test results were recieved on 1/27/2024.
2- Staff #5 hire date was 03/04/2024 , the TB results were recieved on 3.07/2024.
3-The TB tests were not completed within 30 days prior to employment.
Plan of Correction: I understand regardless of a potential employee they will not have a hire date before all acceptable required documents have been received and verified.
