Inspector Notes

A non-mandated monitoring inspection was initiated on conducted on December 4, 2023 from 10:15 A.M.-12:30 P.M. There were seven children present, ranging in ages from one year six months to five years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-500-B
Based on observation, the center failed to ensure that staff followed proper procedures during diaper disposal.

Evidence:
1. In the Toddler Classroom, Staff #2 completed a diaper change and did not use the foot pedal to dispose of the soiled diaper. Staff #2 lifted the lid of the trashcan with her hand to dispose of both the diaper and gloves.
2. Staff #2 verified that they did not use the foot pedal to open the trash can to dispose of the soiled diaper.
Plan of Correction: Staff have briefed on diaper changing procedures and plan to use the foot pedal consistently.
Standard 8VAC20-780-510-P
Based on a review of medication and medication documentation, the center failed to ensure that medications were not picked up or disposed of within 14 days of when the authorization for medication expires.

Evidence:
1. Child #1 had an over the counter medication, (Children's Motrin), on site. The medication authorization form expired on 10/09/2023. The medication had not been administered.
2. Administration verified that the medication authorization form was the most current, and that the parents had not been notified to pick up the medication or disposed of within 14 days of expiration of the authorization form.
Plan of Correction: Checked with parents to get the form updated or to have the medication returned. If the parents choose not to update or take the medication back. GMS staff will dispose of the medication properly.
