Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 03/26/2024, between the hours of 10 am to 1 pm. There were 45 children present, ranging from infant to school-age, with 11 classroom staff supervising across 7 groups. The children were engaged in the following: learning, center activities within their classroom, diapering, hand washing, snack, music, circle time, computers, bathroom breaks, lunch, nap time, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 7 children files, 2 board officer records, and 15 staff files were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Standard 8VAC20-780-450-A
Based on observation, 3 of 4 cribs had bottom covers that were not the appropriate size for the mattress to be used.

Evidence -
1. On the date of inspection (03/26/24), within the Infant II room, bottom sheets appeared to be "bunched" on its surface and not taught; indicating that the sheet was too large for the size of the mattress.
2. These lines were provided by each child's parents.
Plan of Correction: Parents will be notified. Cribs sheet will be purchased and replaced by the CDC.

Based on review, 1 of 7 children files indicated that the documentation of health information as required by 8VAC20-780-130 (immunization page), and 8VAC20-780- 140 (physical).

Evidence -
1. On the date of inspection (03/26/2024), health forms were not available for review for Child #3.
2. The start date for Child #3 was listed as 08/05/2022.
Plan of Correction: Parent was contacted. Obtained health forms will be placed on file.
