During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/25/2024. The on-site inspection began at 10 am and ended at 12 pm. The inspector reviewed compliance in the areas listed above. There were (28) children present and (8) staff. The inspector reviewed (5) children?s records and (1) new staff record on-site on 10/25/2024.

This inspection included: document review (files, policies and procedures, emergency drill logs, no medications/ no allergies, authorization forms); tour of the facility; and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
A physical examination completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the child's first day of attendance.

There was not a physical examination in the record of child #2, who had been enrolled for two months.
Plan of Correction: A physical will be obtained and placed on file.
Standard 8VAC20-780-160-A
The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #8 is currently working and has a TB screening that was dated 2 days after their first day of employment.
Plan of Correction: TB documentation will be obtained and placed on file.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A spray bottle of a water/bleach mix was observed unlocked in the Pre-K classroom, within reach of children.
