During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/24/2025. The on-site inspection began at 10 am and ended at 12 pm. The inspector reviewed compliance in the areas listed above. There were (33) children present and (10) classroom staff. The inspector reviewed (5) children?s records and (14) staff records on-site on 09/24/2025.

This inspection included: document review (i.e. Files, injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms); tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #4, who had been enrolled for nine months.
Plan of Correction: This documentation shall be obtained and placed on file.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #8 TB was dated four months prior to date of employment and has been employed for three months.
Plan of Correction: Going forward, the center shall be in compliance with this standard and obtain TB documentation no older than 30 days prior to the staff person's start date.
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.

The fall zones around the half globe climber, located on the back left corner of the center playground had grass and weeds growing through the resilient surface (wood chips) provided.
Plan of Correction: We shall re-rake the resilient surfacing to have compliance in all fall zone areas. Both the grass and weeds shall be removed and/or covered to the appropriate depth.
