Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/5/2024. The on-site inspection began at 1:50 p.m. and ended at 2:40 p.m. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 22. The inspector reviewed 5 children?s records and 2 caregiver records. This inspection included document review, a tour of the family day home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
11. Child #1 and child #4 records did not contain the families medical insurance policy numbers.
2. Child #3 record did not contain two emergency contacts.
