Inspector Notes
An unannounced, on-site monitoring inspection was initiated on November 26, 2024 and completed on November 26, 2025. The on-site inspection began at 9:15 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 36 children present and 12 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4Staff #1 who has been employed for 41 days, did not have central registry results. Staff #1 was working alone with children.
On the playground one wooden playhouse had a screw sticking out toward the inside of the playhouse. A second wooden playhouse had rough wood edges within reach of children.
Child A, child B, and child C had emergency medication at the center. The medication was not labeled with the child?s name, the name of the medication and dosage amount, and times to be given.
Child A, child B, and child C did not have a written care plan for a diagnosed food allergy.
