Inspector Notes

An unannounced, on-site monitoring inspection was initiated on November 26, 2024 and completed on November 26, 2025. The on-site inspection began at 9:15 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 36 children present and 12 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day of employment.

Staff #1 who has been employed for 41 days, did not have central registry results. Staff #1 was working alone with children.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

On the playground one wooden playhouse had a screw sticking out toward the inside of the playhouse. A second wooden playhouse had rough wood edges within reach of children.
