Inspector Notes

A monitoring focus inspection was conducted today between 11:00am and 1:00pm. There were 41 children present, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed eating lunch and getting ready for nap, and a diaper change was observed. A total of 5 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of children's records, the center did not obtain a physical examination within 30 days of first day of attendance.
Evidence
The record for child #1 start date 10/16/2023, does not have a complete physical in their record.
Plan of Correction: I will contact the parent and get a copy of the complete physical.
Standard 8VAC20-780-240-B
Based on review of staff records, the center did not ensure staff completed orientation training within seven days of the date of assuming job responsibilities.
Evidence
Staff #1 and #2, first day of employment 10/30/2023, do not have documentation of completing orientation within seven days of the date of assuming job responsibilities.
Plan of Correction: I will get the documentation of the completed orientation and place it in each of their records.
