During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on December 2, 2024. The on-site inspection began at 10:30am and ended at 11:45am.The inspector reviewed compliance in the areas listed above. There were 97 children present and 25 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and interviews, and observations.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Bleach and water was not locked up in the Infant and Toddler classrooms.
Laundry detergent and bleach was not locked up in the hallway closet.
Plan of Correction: The bleach bottles are kept up high out of reach of children, we will make sure they are kept locked as well.
We will make sure the hallway closet is always locked.
Standard 8VAC20-780-40-J
Injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services

The center has not updated the injury prevention procedures.
Plan of Correction: We will begin today keeping an injury prevention plan to review annually.
Standard 8VAC20-780-500-A
Staff shall wash their hands with soap and running water before and after diapering a child.
Staff did not wash their hands after diapering a child in the Infant classroom
Plan of Correction: The staff used hand sanitizer after the diaper change, but we will review the proper procedures again.
Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #1 cleaned the diaper changing surface but did not sanitize the surface.
Staff #2 cleaned the diaper changing surface but did not spray with a solution of bleach and water to sanitize the surface.
Plan of Correction: We use a paper on the diapering surface that is changed after each diapering, but will make sure to sanitize after each diaper change as well.
