During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 9/5/2025, as a part of the licensure period. The on-site inspection began at 9:20 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were (107) children present and (30) staff. The inspector reviewed (10) children?s records and (10) staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The list in the classrooms are posted but have not been dated.
Plan of Correction: The allergy postings posted in the classrooms will be dated.
Standard 8VAC20-780-510-E
Long term prescription medication use may be allowed with written authorization from the parent and the child's physician. The center shall include methods to prevent outdated medication.
There was no written authorization from the child's physician to administer prescription medications to child # 8.
The medication for child # 7 expired 8/2025.
Plan of Correction: The parent will obtain a written physician authorization for the medication.
The parent will provide a current medication.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child #6 was onsite, but the medication authorization had expired over 12 months ago. The authorization had not been renewed and the medication was not returned or disposed of as required.
Plan of Correction: The parent will provide a current medication authorization form.
