During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, renewal inspection was completed on March 10, 2025 The inspection began at 12:05 PM and ended at 2:15PM. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was 15. The inspector reviewed four children?s records and five caregiver/household and staff members records. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations (this would include observations of photos, recordings, etc.), and
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
IF VIOLATIONS ARE CITED, AND A POC HAS NOT BEEN RECEIVED, ADD THIS PARAGRAPH: Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Violations
6Staff #1 has bee employed for 5 months and did not have documentation of a tuberculosis screening in their record.
Child #1 was missing the addresses for emergency contacts and work phone numbers for both parents. The child has been enrolled for two months.
Child #2 was missing one parent's work phone number and address. The child has been enrolled for one month.
The diapering pad had scratches and cracks making it absorbent.

Practice emergency evacuation procedures have not been conducted in three months.
The provider did not conducted one of the two required shelter-in-place drills within the year.
The temperature of the refrigerator was 55 degrees and freezer was 15 degrees.
