During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/12/2024. The on-site inspection began at 12:25pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 31 children present and 10 staff. The inspector reviewed 5 children?s records and 4 staff records on-site on 12/12/2024. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
12Staff are required to submit to fingerprinting prior to employment.
Staff #4 had fingerprints that were completed two days after employment began.
The Child day center is required to obtain a copy of the results of a criminal history record
information check, a sex offender registry check, and a search of the child abuse and
neglect registry or equivalent registry from any state in which an individual (employee) has
resided in the preceding five years.
Staff #2 did not have results of an out-of-state Central registry check and sex offender registry
check for a state in which the employee has lived in the preceding 5 years and has been
employed for 8 months.
The center is required to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #3 did not have documentation of immunizations on file at the time of the inspection. The child had been enrolled for 5 months.
Each child is required to have a physical examination by or under the direction of a physician by day 1 of enrollment or within 30 days of enrollment.
Child #3 did not have documentation of a physical examination on file at the time of the inspection and had been enrolled for 5 months.
Staff #2 did not have documentation of completing The Virginia Department of Education-sponsored orientation course on file at the time of the inspection. Staff #2 had been employed for 8 months.
Staff are required to complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
There was no documentation of orientation training on file for Staff #3 at the time of the inspection. Staff #3 had been employed for 1 month.
Staff #2 did not have documentation on file of completing orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care at the time of the inspection. Staff #2 had been employed for 8 months.
There were 3 emergency medications stored in a backpack that was within reach of children in one of the classrooms at the time of the inspection.
The emergency radio on-site was not in working condition at the time of the inspection.
There was no documentation of evacuation drills being practiced for March, June and October of 2024.
There were 8 bottles of infant formula with no date on them and 2 bottles of infant formula with no name on them at the time of the inspection.
It is required that documentation of two or more references as to character and reputation as well as competency were checked before employment and kept for each staff person.
Staff #3 did not have documentation of two or more required references on file at the time of the inspection. Staff #3 had been employed for one month.
