During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/10/2025. The on-site inspection began at 11:40am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 40 children present and 12 staff. The inspector reviewed 5
children?s records and 4 staff records on-site on 6/10/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

**REPEAT VIOLATION**
The Child day center is required to obtain a copy of the results of a criminal history record
information check, a sex offender registry check, and a search of the child abuse and
neglect registry or equivalent registry from any state in which an individual (employee) has
resided in the preceding five years.

Staff #3 did not have results of an out-of-state Criminal history record information check, Central registry check and sex offender registry check for a state in which the employee has lived in the preceding 5 years and has been employed for one year.
Plan of Correction: We will complete the out-of-state background checks for the employee within 10 days as required.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor is required to submit documentation of a negative tuberculosis screening by day one of employment or no more than 30 days prior.

Staff #1 did not have a current tuberculosis screening on file by day one of employment or no more than 30 days prior.
Plan of Correction: We will have the employee get a TB screening and place it in the file.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, are required to be maintained in a clean, safe and operable condition.

The air condition on the second floor was not working. There were small window air condition units temporarily placed in two of the windows in two of the upstairs classrooms that were not secure. The only toilet in the Cruisers 2 classroom was not in working order. There was a floor seat in the toddler room that was torn across the bottom with foam coming out.
Plan of Correction: The broken seat pad was removed from the classroom during the inspection.
Standard 8VAC20-780-290-A-2
Fans, when used, are required to be out of reach of children and cords shall be secured so as not to create a tripping hazard.

There was a fan in the Pre-K room that was located on the floor within reach of the children and the cord was not secure.
Plan of Correction: The AC was fixed so we will remove the fan.
Standard 8VAC20-780-440-L
Pillows and filled comforters shall not be used by children under two years of age.

There were 3 infants observed with blankets in their cribs and 1 of the infants had a pillow in the crib as well.
Plan of Correction: The pillow was removed from the crib during the inspection. We will not use pillows moving forward.
Standard 8VAC20-780-500-B
The diapering area is required to have a nonabsorbent surface for diapering or
changing. For children younger than three years, this surface shall be a changing table or
countertop designated for changing.

The changing pad used for diapering in the Infant and Cruisers 2 classrooms had multiple rips in them and were no longer nonabsorbent.
Plan of Correction: We will order new changing pads and replace the ones with tears/rips.
