**REPEAT VIOLATION**
Staff are required to submit to fingerprinting prior to employment.

Staff #4 did not submit to fingerprinting prior to employment as required.
Plan of Correction: We will make sure we conduct the fingerprints background check before the date of hire for an employee.

**REPEAT VIOLATION**
The Child day center is required to obtain a copy of the results of a criminal history record
information check, a sex offender registry check, and a search of the child abuse and
neglect registry or equivalent registry from any state in which an employee has resided in the
preceding five years.

Staff #2, Staff #3 and Staff #4 did not have documentation of the results of an out-of-state
search of the child abuse and neglect register for a state in which the employees have resided in the preceding five years.
Plan of Correction: We will send the missing out-of-state background check requests out by Friday 12/5 and we will follow-up with the results to licensing as required within 10 business days.

**REPEAT VIOLATION**
Each staff member and individual from an independent contractor is required to submit
documentation of a negative tuberculosis screening by day one of employment or no
more than 30 days prior.

Staff #4 did not have a current Tuberculosis screening on file at the time of the inspection.
Plan of Correction: We will have the staff member get a new TB screening by 12/12.

Areas and equipment of the center, inside and outside, are required to be maintained in a
clean, safe and operable condition.

There was peeling paint observed on multiple walls in multiple areas in the Toddler, Cruisers 1, and Infant classrooms.
Plan of Correction: We will speak to the owners to see if we can get approved to have all of the classrooms repainted.

Staff shall wash their hands with soap and running water before and after a diaper change.

One staff member was observed changing two children's diapers and did not wash their hands with soap and running water before and after each diaper change.
Plan of Correction: We will retrain the staff on proper procedures today.

**REPEAT VIOLATION**
The diapering area is required to have a nonabsorbent surface for diapering or changing. The diapering surface is required to be cleaned with soap and at least room temperature water and sanitized after each use.

The diapering surface in one of the classrooms was ripped and is no longer a nonabsorbent surface. There was no cleaning or sanitizing of the diapering surface observed in between two diaper changes that occurred in one of the classrooms during the inspection.
Plan of Correction: We will retrain the staff on proper procedures and we will order a new changing pad today.

**Repeat Violation**
Staff records are required to be kept for each staff person including documentation of 2
references of character and reputation taken prior to hire.

Staff #2, Staff #3 and Staff #4 did not have documentation of 2 out of 2 required
references during the inspection.
Plan of Correction: We will get the missing references by 12/12.
