During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation
Inspector Notes
An unannounced, on-site monitoring inspection was conducted on December 16, 2024 from 2:15 p.m. to 4:45 p.m. The inspector reviewed compliance in the areas listed above. There were 24 children present and 8 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment. Staff 2 and Staff 3 who have been employed for 4 months, did not have a central registry search on file. The staff were not working under the supervision of staff with background checks at all times.
Plan of Correction: Center director will contact Central Registry to check on the CPS checks.
Standard 8VAC20-780-240-A
Staff shall complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Staff 4, employed for four months, did not have documentation of having completed the VA Dept of Education sponsored orientation course within 90 days of employment.
Plan of Correction: New staff will get complete the VA Preservice within 90 days of employment.
Standard 8VAC20-780-70
Two references as to character, reputation, and competence, shall be checked prior to employment. If a reference check is taken over the phone, documentation shall include dates of contact, names of persons contacted, the firms contacted, the results, and signature of person making call. Phone references for Staff 1, 2, 3, and 4 did not include dates of contact, the firm contacted, or the results.
