At least every two years from the date of the first initial screening or testing, staff members shall submit the results of a follow-up tuberculosis screening. Staff 2 did not have a current tuberculosis test or screening; the documented test expired 2 months ago.
Plan of Correction: TB tests and screenings will be repeated every 2 years as required.

Documentation of orientation training shall be kept by the center in a manner that
allows for identification by individual staff member, is considered part of the staff
member's record, and shall include: the date of training. Orientation for Staff 3, employed for almost 3 months, was not dated.

The facility shall have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child 1 has diagnosed food allergies but did not have a written allergy action plan on file.
