Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 7/30/2024 and completed on 7/30/2024. The on-site inspection began at 9:40 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 29 children present and 5 staff. The inspector reviewed 6 children?s records and 7 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/8/2024 . A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-140-A
Each child's record shall have a physical examination by or under the direction of a physician within 30 days of enrollment. Child 5's record did not contain documentation of a physical examination.
Plan of Correction: Ask parents to provide signed copy of physical. Administrators will make sure we receive all information moving forward.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff 2, 3, and 4's records do not contain documentation of orientation. Staff 5 received an orientation 27 days after the listed hire date.
Plan of Correction: We will meet with each of the teachers missing orientation and have it completed. We will be more mindful moving forward. Admins will be responsible.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in an operable condition. A sensory board on the toddler playground was observed with ripped felt.
Plan of Correction: We removed fabric from play equipment. Staff will do daily playground checks moving forward.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall require long-term medication to have a written authorization from the child's parent and physician. Child 5's medication with has a written authorization from the parent for 7/25/2024-8/15/2024, but no written authorization from a physic an.
Plan of Correction: We will ask the family to fill out a new form before use. When accepting forms we will check closely to make sure it has proper dates.

The center shall implement a monthly practice evacuation drill. An evacuation drill was not conducted in June 2024.
Plan of Correction: Moving forward we will complete monthly drills.
Standard 8VAC20-780-60-A
The center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information. Child 1's record did not contain addresses for the two listed emergency contacts. Child 5's record contained information for one of the two required emergency contacts.
Plan of Correction: Notified both families to obtain info. We will check children's files more frequently and with a closer eye.
