During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An announced monitoring inspection was initiated on February 18, 2025 and completed on February 18, 2025 with a local agency in response to a self-report received by the licensing office on February 7, 2025 relating to supervision. The inspector reviewed 1 child's record and 3 staff records [on-site].. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 28, 2025. A POC submitted after this date will not appear on the public website.

Violations

Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom provided that: There is a system to assure that individuals who are not staff members or
persons allowed to pick up a child in care do not enter the restroom area while in
use by children; and Staff check on a child who has not returned from the restroom after five
minutes.

1. On February 7, 2025 Child 1 (aged 3 years) was left in a classroom restroom without supervision from approximately 3:22pm until 3:50pm, approximately 28 minutes.
1. Staff 1 and Staff 2 were present in the classroom at the start of the incident and were aware Child 1 was in the bathroom.
2. Staff 1 was relieved by Staff 3 while Child 1 was in the bathroom.
3. Child 1 was in the bathroom for thirteen minutes without a staff member checking up on them when the remaining students, Staff 2 and Staff 3 took the children to the playground.
4. Child 1 remained in the classroom bathroom in the empty classroom until two parents, (including Child 1's parent) entered the room and found the child in the bathroom at approximately 3:50pm.
Plan of Correction: We implemented new bathroom signs in the classroom to indicate when it's vacant or occupied. Staff 2 terminated effective 2/20/2025.
