Inspector Notes

An unannounced non-mandated monitoring inspection was started and ended on 1/23/24 from 2:15 until 3:45. There were 3 children for a total of 10 points and one provider present.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 provider record and one household member's record was reviewed and three children?s records were reviewed. Children observed resting, playing, and having a snack.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the violation notice issued to the program.

Based on record review, there was no central registry search for a 14 yr. old residing in the home.
Evidence:
Child #1 birthdate is 9/29/2007 and there was no central registry available.
Plan of Correction: The provider will get the central registry check completed.

Based on record review, children did not have all the required elements in the child files.
Evidence:
Child #1 was missing names and addresses for emergency contacts, information on allergies and intolerances to food, medication, or any other substances, and actions to take in an emergency situation. Child #2 was missing phone #'s and addresses for emergency contacts, and information on allergies and intolerances to food, medication, or any other substances, and actions to take in an emergency situation
Plan of Correction: The provider will get the information from the parents and make sure records are complete.

Based on observation, medications were not kept locked.
Evidence:
There was a bottle of liquid Dimetapp and Ibuprophen in the black storage cabinet. The cabinet was closed and latched but not locked.
Plan of Correction: Corrected during the inspection.
