During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site inspection was initiated and completed on 05/13/2025. The on-site inspection began at 11:30 and ended at 1:00. The inspector reviewed compliance in the areas listed above. There were 6 children and 2 caregivers present for total of 16 points. The inspector reviewed 5 children?s records and 4 caregiver/household member records. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-770-40-D-5
A person 14 years of age and older must have a search of the central registry and make the information available for regulatory purposes who is living in the licensee's home.

Household member #3 has turned 14 yrs.old and did not have a central registry check completed.
Plan of Correction: The central registry check will be completed.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Caregiver #2 was past due for the repeat TB screening by 6 weeks.
Plan of Correction: The caregiver will get the TB screening completed.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Staff person #2 had not completed the required 16 hours of annual training.
Plan of Correction: The caregiver will complete the required training.
Standard 8VAC20-800-830-A
This is a repeat violation.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

The provider did not have documentation of monthly evacuation drills for October, November and December.
Plan of Correction: Provider will complete extra drills and make sure monthly drills are completed.
