During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced on-site inspection was initiated and completed on 05/13/2025. The on-site inspection began at 11:30 and ended at 1:00. The inspector reviewed compliance in the areas listed above. There were 6 children and 2 caregivers present for total of 16 points. The inspector reviewed 5 children?s records and 4 caregiver/household member records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4Household member #3 has turned 14 yrs.old and did not have a central registry check completed.
Caregiver #2 was past due for the repeat TB screening by 6 weeks.
Staff person #2 had not completed the required 16 hours of annual training.
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The provider did not have documentation of monthly evacuation drills for October, November and December.
