During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/6/25. The on-site inspection began at 10:50am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were seven children present and three caregivers; the point total was 16. The inspector reviewed four children?s records, three caregiver records, and two household member records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/14/25. A POC submitted after this date will not appear on the public website.

The family day home is required to keep a written record of children in attendance each day.

On 8/6/25 at approximately 10:50am, there were seven children present in the daycare. The attendance record contained documentation of eight children present.
Plan of Correction: We will be more careful recording the daily attendance.
Standard 8VAC20-800-90-C
The family day home is required to obtain documentation of additional immunizations for children once every six months for children under the age of two years; and once between each child?s fourth and sixth birthdays.

Additional immunizations were past due for Child A, Child B, Child C, and Child D.
Plan of Correction: We request to parents update the immunizations records for their children.
