Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 9/16/2025. The inspection started at 9:30 am and concluded at 10:35 am. There were 4 children present and 1 caregiver; the point total was 10. The inspector reviewed 4 children?s records and 4 caregiver/household member(s) records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Caregivers shall undergo TB screenings at least every two years from the date of the initial screening. Caregiver #1 TB screening was seven months past due.
