Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 9/16/2025. The inspection started at 9:30 am and concluded at 10:35 am. There were 4 children present and 1 caregiver; the point total was 10. The inspector reviewed 4 children?s records and 4 caregiver/household member(s) records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
