During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 9/11/2025 and completed on 9/18/2025 in response to a complaint received by the licensing office on 9/8/2025 relating to allegations of supervision and forbidden actions. The inspector reviewed one child?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to program records, and are documented on the violation notice issued to the program.

A substitute provider shall record and sign the time of arrivals and departures
on each day that the substitute provider works. The substitute provider sheet was not updated with the arrival and departure of the substitute provider at the time of the inspection.
Plan of Correction: The provider will update the substitute provider tracking sheet.
Standard 8VAC20-800-920
When meals or snacks are provided by the family day home, the menu for the
current one-week period shall list any substituted food. The posted menu was not updatd with updated food from that day.
