Inspector Notes
A renewal inspection was conducted on 04/30/2024 from approximately 12:25 p.m. to 1:40 p.m. There were 6 children present and 1 staff supervising. The inspectors reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Six child (6) records and one (1) staff record were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). The staff record for S2 did not contain documentation of orientation.
Evidence:
Licensing inspectors observed S1 alone with six children on the premises. S1 reported that the classroom assistant S2 was terminated 04/19/2024.
Evidence:
S1 reported to licensing inspectors that C6?s medication is maintained in the child?s backpack in the cubbies at the front door of the center. S1 acknowledged the medication was not in a locked place using a safe locking method that prevents access by children.
Based on review of emergency evacuation, shelter-in-place, and lockdown drills documentation, the provider failed to ensure the documentation contained the required
information.
Evidence:
The emergency drill documentation dated February, March, and April 2024 did not document: the person conducting the drill; the method used for notification of the drill; the
number of staff participating; the number of children participating; any special conditions simulated; the time it took to complete the drill; problems encountered, if any; and weather conditions.
Based on review of children records, the provider did not ensure three of six
children?s records contained the required information.
Evidence:
The records for children, C4, enrolled 03/04/2024 and C6, enrolled 03/11/2024 did not contain documentation of viewing proof of the child?s identity and age.
The record for child, C4 enrolled 03/04/2024 did not document a second designated person
to contact in case of an emergency if the parent cannot be reached.
The record for C1, enrolled 04/08/2024 did not contain immunizations required by 8VAC20-780-130.
