Inspector Notes
This initial inspection was initiated by licensing staff and concluded on 10/17/2023, between the hours of 9:45 am to 1 pm. There were 3 preschool children present (6 points), with 1 caregiver supervising. The children were engaged in the following: learning, games, center activities, outside playtime, bathroom breaks, hand washing, getting ready for lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 3 children records, 1 staff record, and 1 household record was reviewed.
Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the home.

4Evidence -
1. On the date of inspection (10/17/23), a search of the child abuse and neglect registry was not available for review for a state (having resided their within the past 5 years) as indicated on the completed sworn statement (dated - 09/12/23) for Household Member #1.
2. The Provider stated (during an interview on the date of inspection) that both requests for findings were sent at the same time. However, white the finding for Staff #1 came back, the finding for Household Member #1 has not.
3. All other background checks have come back and were in compliance.
Evidence -
1. On the date of inspection (10/17/2023), the place of employment for each child's parent was not listed for: Child #1 (start date - 09/16/23), Child #2 (start date - 09/05/23), and Child #3 (start date - 09/05/23).
2. The addresses for each listed emergency contact were not available for: Child #1, Child #2, and Child #3.
Evidence -
1. On the date of inspection, the following children did not have a completed Medication Administration - Decision to Administer form within their file - Child #1, Child #2, and Child #3.
2. The following children did not have a completed Provisions of the Emergency Preparedness and Response Plan form within their file - Child #1, Child #2, and Child #3.

1. On the date of inspection (10/17/2023), documentation in each child's record that verification of the proof of identification was reviewed for each child was not available for: Child #1, Child #2, and Child #3.
2. The Provider stated (during an interview conducted on the date of inspection) that she viewed the birth certificate for each child, but did not document these reviews.
