During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/18/2025. The on-site inspection began at 10 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (4) children present and (1) caregiver; the point total was (8). The inspector reviewed (5) children?s records, (1) caregiver record, and (1) household member record on-site on 09/18/2025.

This inspection included: document review (i.e. Files, injury logs, policies and procedures, and emergency drill logs); tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the most current documentation on file.
Both Staff #1 and Household Member #1 had TB documentation on file that expired one month ago.
Plan of Correction: Will obtain updated TB documentation for both individuals and place it on file.
Standard 8VAC20-800-70-B
Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration.

There was no parent acknowledgement (Medication Administration - Decision to Administer form) available for one child who was provided care in the past two weeks.
Plan of Correction: Will get signed form from parent and place it on file.
