Inspector Notes

An unannounced monitoring inspection was initiated on February 13, 2024 and concluded on February 21, 2024. There were 19 children in care ages infant through four years old. There were five staff and one administrator present during the inspection. A walking tour of the center inside and outside was completed. Children were observed teacher and self-directed activities including but not limited to nap, transitions, and restroom routines. Violations regarding unlocked chemicals, diaper changing, infant bottles, physical plant, staff records, emergency evacuation drill documentation and emergency equipment were observed and discussed with the program director during the exit meeting at the conclusion of the inspection.

Based on observation the center failed to ensure that areas are maintained in safe and operable condition.
Evidence:
The door leading from the toddler care room to the outside is very difficult to open and close and may pose a hazard during an emergency.

Based on observation the center failed to ensure that hazardous substances are kept in a locked place.
Evidence:
In the infant care room there were cleaning and sanitizing substances stored on top of a cabinet, which was an unlocked place.

Based on observation the center failed to ensure that staff wash their hands before changing diapers.
Evidence:
A staff in the infant room did not wash her hands before changing the diaper of a child in care.

Based on observation the center failed to ensure that the diaper changing surface is sanitized after each use.
Evidence:
After changing the diaper of a child in care a staff sprayed the diaper changing surface with sanitizer and immediately wiped it dray without allowing time for the surface to air dry as required by definition of sanitized.

Evidence:
The center began operation on November 22, 2023. However, there was not any documentation that emergency evacuation drills had been practiced since the facility opened.
