During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 10/10/24. The inspection started at 9:20am and concluded at 10:25am. There were two children present and one caregiver; the point total was eight. The inspector reviewed two children?s records and five caregiver/household member records on-site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/24/24.

Standard 8VAC20-790-230-A
Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized according to the requirements of the Code of Virginia and State Board of Health regulations.

Child #1 was enrolled for over one week before documentation of immunizations was obtained. Child #2 was enrolled for over six months before documentation of immunizations was obtained.
Plan of Correction: Immunization documentation was obtained for both children prior to enrollment, the original documents were inadvertently replaced with the most up-to-date versions to avoid duplicates. This was an oversight on the provider's part. The daycare provider will implement a new protocol for managing immunization records that includes retaining both original and updated documents in each child's file.
