During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 2/10/2025 and completed on 2/10/2025. The on-site inspection began at 9:30 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were (12) children present and (4) staff. The inspector reviewed (5) children?s records and (4) staff records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/14/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #1, who had been attending for one month, did not have documentation of an immunization record on file.
Plan of Correction: Corrected, obtained record from parent.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #1, who had been enrolled for one month. There was no physical examination in the record of child # 4, who had been enrolled for five months.
Plan of Correction: Corrected, obtained record from parent.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The list was not posted in the 2 year old room and the nap room.
Plan of Correction: We made an allergy list for all classes.
Standard 8VAC20-780-510-E
Long time prescription drug use and over the counter medication may be allowed with written authorization from the child's parent and physician.
There was no written authorization to administer medication from the parent and physician for child # 6 and for child # 7. Both children had medication on site.
Plan of Correction: Gave the medication back to the parents. We have never used the medications.
Standard 8VAC20-780-510-I
The center may administer prescription medication to the child identified on the prescription label in accordance with the prescribers instructions pertaining to dosage, frequency and manner of administration.
There was no prescription label on child # 6 medication.
Plan of Correction: Gave medication back to parent.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
At approximately 9:40 a.m. on 2/10/2025, there were 4 children present in the twos room, there was no record of daily attendance.
Plan of Correction: Corrected. We continue to keep records in each class.
