Inspector Notes

An unannounced, onsite monitoring inspection was initiated and completed on 11/12/2024. The on-site inspection began at 9:30 am and ended at 11:25 am. The inspector reviewed compliance in the areas listed above. There were 25 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The immunization records for Child 1 (7 months), who has been attending for 5 months, were not updated once every six months. The immunization records for Child 4 (15 months), who has been attending for 10 months, were not updated once every six months. The immunization records for Child 5 (23 months), who has been attending for 8 months, were not updated once every six months.
Plan of Correction: Will ask the parents for updates.

In areas used by preschool age or younger children, electrical outlets shall have protective covers.

There were 3 uncovered electrical outlets in area used by children ages 2 years-4 years.
