During the inspection, the licensing representatives reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.?
Inspector Notes
An unannounced, onsite monitoring inspection was initiated and completed on 10/22/2025. The on-site inspection began at 10:30 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 5 staff. The inspector reviewed 5 children?s records and 4 staff records.
The inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3The molding at the base of the wall in the women?s restroom is peeling off. The paint on the stalls in the men?s restroom is peeling and the stall walls are rusting. There is a loose floorboard in Classroom 3.
There was a child using an uncovered pillow during rest time in Classroom 3.
Staff records shall contain all of the elements as required by the standards.
The record for Staff 1 and Staff 2 did not contain documentation to demonstrate that the individual possesses the education and experience required by the job position.
