Inspector Notes

An initial subsidy health and safety (SHSI) inspection was initiated and concluded on 11/13/2023. There were 83 children present with 19 staff supervising and 2 administrators. Children were observed during nap time, clean up time, and teacher led activities. A total of 5 children and 5 staff records were reviewed. The inspection started at 11:55 am and concluded at 1:15 pm.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on documentation review and staff interview the center did not ensure staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: The director confirmed that the files for Staff Members 1-5 (dates of hire 9/21/2023) did not contain documentation of a TB screening prior to coming into contact with children.
Plan of Correction: It was a confusion in interpretation in reading the website and all screenings will be completed by 12/4/2023.
